Yes, it is fucked, but it’s absolutely the case - especially in places that don’t have informed consent where access to hormone treatment is harder to get. In my country, I and many other people have had to lie repeatedly to doctors in order to fit their perfect image of a trans woman - I imagine wanting to continue using he/him pronouns would be a huge red flag for these doctors.
