When something other than your booger hook hits the bang switch, or when your booger hook hits the bang switch but you didn’t want it to.

Most common way to shoot yourself in the foot is to draw your gun, do what needs doing, then fuck up royally when trying to put it back in the holster; either some loose clothing winds up inside the trigger guard, our your finger wasn’t as out of there as you thought it was. Said item gets stuck on the holster, and yanks the trigger, firing a gun that was pointed pretty much directly at your foot.

Second most common is fucking up royally on the draw itself, and putting your finger on the trigger before the gun is pointed in the intended direction. This was/is extremely common with the Blackhawk SERPA holster, which uses a lock that is toggled by the trigger finger, exactly where the trigger is, by design.
